No model connected

No model is connected. Set ANTHROPIC_API_KEY to enable note drafting and visit summaries. Result explanations and triage do not use a model and work either way.

Identifiers are stripped before anything is sent, and what is stored in this log is what actually left. Nothing generated becomes part of a record until a clinician accepts it.

This log has no preview fixtures on purpose. A plausible audit trail is the one thing this table must never contain, so it is empty until it is real.

This table has been in the schema since the first migration. Provenance on generated text cannot be added afterwards: backfilling which model wrote a note, and whether a human checked it, from rows that never recorded either produces a column of nulls that answers nothing.
